Through a holistic and anticipatory framework, this project addresses aftercare support for lymphedema and wound care patients in the NHS at the Pioneer Wound Healing & Lymphedema Centres. The initiative leverages a digital platform alongside innovative future planning workshops to create sustainable long-term care solutions and enable quick service improvements for the immediate present. By integrating patient experience, psychological care, and healthcare delivery into a cohesive ecosystem, this project establishes a new model for transformative healthcare design within the NHS.

Future Roadmap
This is a comprehensive 10-year strategic planning framework organized in two views: by principles and by projects. It maps out Pioneer Centre's transformation from 2025 to 2034, incorporating key milestones, metrics, and initiatives across different timeframes. The roadmap integrates both high-level organizational principles and specific project-based activities to create a clear path forward.
WHY: During the research phase, one major finding was that while Pioneer Centre excelled at operational decisions, there was room for improvement in their strategic planning for long-term improvements. The Future Roadmap was developed to address this gap, helping shift their mindset from short-term solutions to more strategic thinking while providing a structured approach to implementing major changes like the digital platform initiative.
VALUE: This tool offers Pioneer Centre a practical way to envision their preferred future and work backwards to achieve it. The dual view approach (principles and projects) ensures that both strategic vision and tactical implementation are addressed, while providing a shared reference point for all stakeholders to align their efforts and track progress over time.
